Summer Camp Activities

Parents – Please register below for the activities you would like your child to participate in for Summer Camp. Dates, times, and costs are included. There will be additional on-site activities offered during summer camp that are included in the cost of camp. Review the dates carefully before registering to ensure your child will be attending camp during the activities that you choose. Swim Lessons at The YMCA: Experience a progressive level-based format, designed to help children gain confidence and ability in a safe and effective manner. Swim Lessons are offered for children ages PreK-12. The cost per child per session is $75 and this includes transportation and supervision. Please send swim suit and towel on swim lesson days.
Please note: This is DIFFERENT from the regular FREE weekly swimming that happens at the Seashore Water Park.

- July 8: Boone’s Pond Canoe trip – Free
- July 15: Tropicanoe Cove Water Park (Lafayette, IN) – Free
- July 22: Columbian Park (Lafayette, IN) – Free
- July 29: Get Air Trampoline Park (Lafayette, IN) - $28/person

Waiver of Liability*
WAIVER OF LIABILITY & HOLD HARMLESS AGREEMENT
SELECTED FIELD TRIPS
MHABC SUMMER CAMP 2026

Description of Activity:
Travel to locations selected within this form and participation in field trip experience(s) in June and/or July 2026.

Parents: Please read this form carefully and be aware in signing this waiver for you and your minor child(ren)/ward to participate in the activities described above, you will be waiving your rights to all claims for injuries you and/or your minor child/ward might sustain arising out of participating in such activities with MHABC summer camp and agreeing to indemnify, hold harmless and defend MHABC for any claims arising out of your minor child/ward participating in any of the above described activities.

WHEREFORE, in consideration of myself and my minor child/ward being allowed to participate in any of the above-described activities, as the parent or legal guardian of a participant under 18 years of age, I recognize and acknowledge that there are certain risks of physical injury associated with participating in various field trip activities, including but not limited to: play at outdoor and indoor parks, attractions, water parks, and ponds (canoeing). I agree for my child(ren) listed below to participate in said activities described above (which I have selected), and I agree to assume the full risk of injuries that may be sustained by any minor child/ward of mine, as a result of such activities described above, and connected or associated therewith. I agree to waive and relinquish all claims on behalf of my minor child/ward that the minor child/ward may have against MHABC as a result of the minor child/ward participating in any of the said activities.

I do hereby fully release and discharge MHABC and its officers, agents and employees from any and all claims from injuries, damage or loss which I, or any minor child/ward may have or which may occur to my minor child/ward on account of his/her visiting the locations selected and participating in related activities. I further agree to indemnify and hold harmless and defend MHABC, its officers, agents and employees from any and all claims sustained by me or my minor child/ward, and arising out of, connected with, or in any way associated with participating in said activities. The invalidity or unenforceability of any of the provisions hereof shall not affect the validity or enforceability of the remainder of this Agreement.
